A voucher code can only be used once. If you have successfully registered with your voucher code, you can log in with your username and password. If you are sure that you have not registered yet, but your voucher code still does not work, please send an e-mail to firstname.lastname@example.org with the following information:
Date of birth
Party from which you received the voucher code.
The Servicedesk will investigate why your voucher code does not work and if necessary, give you a new one.
If you have lost your voucher code, you van send an email to email@example.com saying you have lost your voucher code and from who you have received your voucher code (employer, health insurer, municipality). If you have already completed your registration, you only need to log in with your username and password. You do not need your voucher code anymore.
You can receive a voucher code from your employer, health insurer or your municipality. They will send it you by e-mail or by post. For more information see the header ‘Expenses’. It is also possible to start the Check privately without a voucher code.
A voucher code is code consisting of 16 characters which gives you access to the Personal Health Check with additional measurements. The code will be given to you by your employer, health insurer or municipality. You can find the code on the invitation you received from one of them.
Without a voucher code it is still possible to gain access to the Personal Health Check. You can pay to participate in the chosen modules.
Some insurers will only cover some of the modules (partially). Check with your insurer what is and is not covered by your insurance. If you choose a module that is not (completely) covered by your health insurer, you will have to pay for the extra expenses.
