The online questionnaire can be done from anywhere in the world. However, for additional modules which require a checkbox, a Dutch address is necessary. If you live outside the Netherlands, please make sure you fill in a Dutch address where you can pick up the checkbox. After you have completed the physical tests, you will also need to send the materials back to the laboratory at a Dutch post office.

You can still log in with your username and password. After you have logged in, you can choose to start a new participation. If you have received one, you can enter your Voucher code here or you can check your health insurance. If you have not received a voucher code, you can participate for a fee. If you have any trouble starting a second participation, please send an e-mail to info@persoonlijkegezondheidscheck.nl.

If you have moved, you can fill in your new address in the settings of your account. The checkbox will be sent to your new address. If it is not possible to change your address, please contact the Servicedesk at info@persoonlijkegezondheidscheck.nl. They will change the address for you.

If you want to delete your account, you can do so through your own account. Another option is to send an e-mail to info@persoonlijkegezondheidscheck.nl with the following information:

 

Full name

Date of birth

E-mail address

 

The Servicedesk will check your personal information before your account gets deleted. The information which cannot be traced back to you will be used for scientific research through the research database.

 

If you want to end your participation with the Personal Health Check, for whatever reason, you can do so by sending an e-mail to info@persoonlijkegezondheidscheck.nl. Please also mention the following:

 

Full name

Date of birth

Reason for ending your participation

 

The Servicedesk will make sure that your participation will be terminated. If you want to participate at a later date, you can log in with the same username and password to start a new participation. It is also possible to end your participation in your account.

If you have completed all the steps to register, you will directly receive a confirmation e-mail at the e-mail address you filled in. If you want to log in at a later moment, but can’t remember what your username is, you can the information in the confirmation e-mail.

You have five attempts to log in. If you enter an incorrect password for a sixth time, your account will be blocked for half an hour. Every attempt to enter your password during those 30 minutes will result in your account being blocked for an extra 30 minutes per attempt. Please wait until after the 30 minutes has passed before attempting to log in again. Alternatively, you can contact the Servicedesk at info@persoonlijkegezondheidscheck.nl to end the blockade. To do so, please send your username, e-mail address and date of birth to the e-mail address above.

Your username and password will remain valid despite having a different employer or health insurer. You can still log in with your current username and password. We would advise you to change your e-mail address if you used your old work e-mail address. You can change your email address in the settings of your account. This way, you will still receive our e-mails at the correct address.

If you forgot your username and/or password, you could request the information at mijn.persoonlijkegezondheidscheck.nl/login. Choose ‘Help with logging in’.

Lost your password, but you still have your username? Then you can change your password yourself. Click on ‘request a new password’ and an e-mail will be sent with which you can choose a new password. Note: the link in the e-mail is valid for 15 minutes. So change your password as soon as you receive the e-mail. If you have not received an e-mail, please check your spambox first.

Forgot your username? Click ‘request your username’. For this we need your date of birth and e-mail address (which you created your account). You will receive an e-mail with your username at the e-mail address known to us.

If you’re still having trouble logging in, we advise you to contact our Servicedesk at 0900-4477383, between 9:00 a.m. and 12:00 p.m.

To make the registration easier, we filled in your e-mail address as your username. If you receive this notification, it means an account already exists with your username. You can log in with your username and your chosen password. Using your existing account, you can participate again.

It is not possible for more people to use the same e-mail address. Every participant needs to use a separate email address.
